Here's where it actually stands

Semaglutide is a lab-made, long-acting copy of GLP-1, a hormone the gut releases after eating. It lowers blood sugar and reduces appetite. Novo Nordisk makes it. In the US it is a prescription medicine, sold as a weekly injection or a daily tablet under the names Ozempic, Rybelsus and Wegovy. The FDA has approved it for type 2 diabetes and for long-term weight management. Some versions are also approved to lower the risk of heart attack and stroke in certain high-risk adults. One version is approved to slow kidney decline in people with type 2 diabetes and kidney disease. Another is approved to treat a scarring liver disease called MASH. Studies in heart failure and alcohol use disorder continue. A large Alzheimer's disease program found no benefit.

Why we checked it

Semaglutide is one of the most-read drug topics we track, and the interest is steady rather than spiking. English Wikipedia's Semaglutide article had 43,496, 42,046 and 38,937 views in July, August and September 2026. PubMed added 205 records in the 30 days to Oct 11 2026. That is close to its prior monthly rate of about 208 (625 in the preceding 90 days). Two independent publishers (Wikimedia, NLM) show sustained attention, but neither shows a spike, so we do not label it "getting hot".

What we know

In adults aged 45 or older with established cardiovascular disease and overweight or obesity but no diabetes, weekly semaglutide lowered the combined rate of cardiovascular death, heart attack or stroke versus placebo (6.5% vs 8.0%; hazard ratio 0.80) over a mean of about 40 months. More people stopped semaglutide because of side effects (16.6% vs 8.2%).

What we don't know

For adults with obesity but no established cardiovascular disease, does semaglutide prevent a first heart attack or stroke?

What would change our mind

If CRAVE (NCT07218354) shows no benefit over placebo on its primary endpoint, we would describe the alcohol-use-disorder signal as not confirmed in a large trial. A clear benefit would move it into "What we know" for veterans with moderate-to-severe AUD.

If ESSENCE part 2 (NCT04822181; estimated primary completion 2029-04) shows no reduction in liver-related clinical events, we would limit the liver claim to tissue (histology) changes. So far the claim rests on histology only. The MASH approval is an accelerated approval: the FDA letter requires a 240-week placebo-controlled trial showing clinical benefit, which matches the design of ESSENCE part 2, so a null result could affect the MASH indication (U.S. FDA (Drugs@FDA), 2025-08-15).

A randomized primary-prevention outcome trial in obesity without cardiovascular disease. A positive or null result would answer the open question above either way.

What still isn't settled?

For adults with obesity but no established cardiovascular disease, does semaglutide prevent a first heart attack or stroke?

SELECT enrolled only people who already had cardiovascular disease. A ClinicalTrials.gov search on 2026-10-11 found no registered randomized trial of semaglutide for primary prevention in obesity, only observational studies.

What would answer it: A randomized placebo-controlled outcome trial of semaglutide in adults with obesity and no prior cardiovascular disease that reports first major cardiovascular events. Observational studies alone would not settle it.

Already settled: in adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced major cardiovascular events versus placebo (SELECT). In early Alzheimer's disease it did not slow decline (evoke/evoke+).

In adults with alcohol use disorder, does semaglutide reduce risky drinking in a large, multi-site trial?Small randomized trials report reductions in some drinking outcomes. A 48-person trial found fewer drinks per drinking day and less craving, but no change in drinks per day. A 108-person trial in people with obesity found fewer heavy-drinking days. A third trial found fewer heavy-drinking days but no change in lab craving or drinks per day. None is large enough to settle the question. What would answer it: The primary result of the VA CRAVE trial (NCT07218354; 622 veterans; primary endpoint a two-level reduction in WHO risk-drinking level; estimated primary completion 2029-03).
